When a resident develops a wound, skin breakdown, sudden change in condition, or requires medical evaluation, your team can initiate a referral. Early evaluation helps prevent complications and allows residents to receive care without unnecessary hospital visits.

Submit a referral through our secure online form, fax, or phone. Our clinical team reviews the referral and coordinates directly with your facility to schedule the visit and gather any relevant clinical information.

Experience

Our provider evaluates the resident on-site, develops a clear treatment plan, and provides written care instructions for staff. Follow-up visits and care coordination are arranged as needed to support healing, resident comfort, and continuity of care.
